Who You Are
You are a dedicated and compassionate individual with a minimum of six months' experience in supporting people with complex needs, such as learning difficulties, complex health needs, or low/medium level needs that challenge mainstream services. You are committed to a person-centred approach and supporting individuals to become active participants in their community. A relevant qualification, like an NVQ level 2, or a willingness to obtain one, is essential.
What the Job Involves
As a Community Support Worker within the Support & Inclusion Team, you will provide essential support to individuals with profound learning disabilities and complex needs. You will work at the Sutton Inclusion Centre to help people become part of their local community by utilizing local resources and building strong community networks. You will deliver tasks and services by following established methods and adapting them to the needs of service users. Your role includes operational responsibilities such as supporting people with personal care, managing medication and meals, and assisting with community activities. Additionally, you will contribute to care plan development, maintain records, and collaborate with carers and other professionals.
