Who You Are

We are looking for an experienced, confident, and motivated practitioner passionate about safeguarding adults, promoting independence, and improving outcomes for vulnerable people. You should possess sound knowledge and experience of Continuing Healthcare (CHC) and have a comprehensive understanding of the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care 2022. A constructive, forward-thinking approach is essential, along with the ability to embrace change, promote high standards, and help shape a positive and inclusive team culture. This role combines expert practice with leadership responsibilities, making it ideal for an experienced social worker ready to deepen their impact and shape professional standards across the service.

What the Job Involves

As an Advanced Practitioner (Adults) with Wirral Council's Adult Social Care service, you will lead high-quality practice across safeguarding, adult protection, and complex casework. You will support people in accessing appropriate funding streams to meet their assessed needs and work as part of a team alongside new Commissioning and Contracts colleagues. Responsibilities include:

  • Managing and supervising staff to identify individuals eligible for Continuing Healthcare
  • Working collaboratively with health professionals and utilizing local funding streams
  • Supporting casework for funded clients, including those approaching review
  • Creating and monitoring data, managing applications, and overseeing cases
  • Collaborating closely with Legal Services on funding applications and decisions
  • Identifying and communicating risk mitigation options with senior management
  • Engaging with providers on best practices for case recording and evidence collation
  • Conducting strength-based holistic assessments and Care Act Assessments
  • Holding a case load of individuals possibly eligible for CHC and completing necessary documentation
  • Completing Mental Capacity Assessments and attending/chairing professional meetings
  • Managing time effectively, coordinating case load, and ensuring timely documentation
  • Monitoring Service Provision and Standards with Registration and Inspection Department
  • Completing CHC Checklists and attending DST/MDTs with Health Colleagues
  • Conducting Community and Placement reviews, promoting independence and empowerment
