Role

Who You Are

A committed Social Worker focused on reviews and reassessments, with the ability to navigate through the CHC Pathway. You possess strong interpersonal skills for engaging with individuals, families, providers, and brokerage to address changes in care and support plans.

What the Job Involves

As a Social Worker, you will manage a service caseload with customers of complex needs and undertake assessments in line with the Care Act. You'll be responsible for statutory duties related to adult care, supervise non-qualified staff, and collaborate with partners to deliver coordinated services. The role involves updating risk assessments, maintaining compliance with professional standards, and participating in team performance enhancements. Digital competency, particularly with Google tools, is essential for modern working practices. You'll contribute to effective communication and relationships both internally and externally, uphold equity and diversity, ensure confidentiality, and advise on service procurement to achieve best value.
