You are a qualified social worker with strong communication, assessment, and case management skills. You possess a sound knowledge of relevant legislation and social care initiatives. You must have the ability to work in the UK legally, with an undergraduate or postgraduate degree in Social Work and registration with Social Work England.
What the job involves
The role involves supporting adults with physical health and social care needs, promoting positive outcomes, and preventing harm. Responsibilities include facilitating assessments and support planning as per the Care Act 2014 and Mental Capacity Act 2005, reviewing service users, managing cases, and conducting safeguarding investigations. You will coordinate with health and social care colleagues for efficient support planning, consider funding paths, and make effective use of resources.
