Who You Are
You are a qualified social worker with extensive experience in mental health. You have a sound understanding of the needs of people with severe mental health problems and the systems in place to support them. You value independence and aim to improve outcomes for individuals through effective management of supported living placements.
What the Job Involves
As a Placement Review and Step-Down Specialist, your primary responsibility will be to assess and review supported living placements in collaboration with Health partners. Your objectives will focus on facilitating step-down processes to optimize independence. You will be expected to use your management experience and expertise in mental health to ensure the effective delivery of services, while working within a multidisciplinary team setting and managing complex needs assessments.
Skills
- Specialist knowledge of mental health and social care models
- Experience with Reablement/Recovery/Strengths-based working
- Knowledge of Safeguarding, social care legislation, Mental Health Act, MCA, DOLS
- Skills in assessment, care planning, and case management
- Ability to build relationships across organizational boundaries
- Excellent verbal and written communication
- Time management and prioritization
- Influencing skills
- Intermediate IT and report writing
- Ability to work under pressure
